Single-Line vs. Double-Line Incision for Revision Double Eyelid Surgery

Learn the differences between single-line and double-line incision techniques for revision double eyelid surgery to lower high creases and manage scarring.

Single-Line vs. Double-Line Incision for Revision Double Eyelid Surgery

I will be posting about single-line and double-line incision techniques for revision double eyelid surgery. I recall posting about what ‘double-line incision’ was about two years ago.

To explain what this is...

If the existing double eyelid line is the brown line, the blue line represents the new line to be created below it.

This method involves releasing the existing adhesions while excising the old scar tissue.

By lowering the crease this way, the double eyelid line becomes a single line when the eyes are closed. This is commonly known as the ‘single-line incision’ method.

Then, what is a ‘double-line incision’?

As shown in the diagram above, the existing double eyelid (brown line) is left in place, and a new double eyelid (blue line) is created while releasing the internal adhesions of the old line.

In this case, there will be two scar lines. This is called the ‘double-line incision’ technique.

You can see the scars in the photo below.

The faint line above is the old scar, and the slightly darker line below is the current scar. Since this is about 4 months post-op, it still appears reddish.

This is a photo of another patient after about a year.

Both the upper and lower scars have faded significantly.

The patient above also achieved improvement through this method.

For this surgery to be feasible:

First, there must be a certain amount of excess skin available. It is only worth attempting if there is enough skin for the eyes to close properly.

Second, it should be at least 6 months since the previous surgery; the longer the wait, the higher the success rate.

Finally, the success rate is higher when the procedure is performed cleanly by an experienced surgeon.
