Looking at the patient’s eyes, one side has a high double eyelid fold, while the other side exhibits multiple overlapping folds.

The visible size of the pupils also differs slightly.

Consequently, the patient’s right eye (left side in the photo) appears sleepy.

In fact, both eyes look somewhat sleepy, but that side appears relatively more tired than the other.

Occasionally, patients with multiple folds ask if they need fat grafting. Personally, I prefer to achieve improvement without fat grafting whenever possible, though I do recommend it in cases where it is absolutely necessary.

In this specific case, fat grafting was not performed.

The surgery involved lowering the fold height slightly using the commonly known ‘double-line extraction’ technique, combined with ptosis correction to address the sleepy appearance.

The key to successful surgery for multiple eyelid folds is thoroughly releasing the existing adhesions!!!!!!!!!!

This is the appearance 8 months after the surgery.

The multiple folds have been resolved, and the sleepy eye appearance has also been corrected.

The core of this surgery lies in properly releasing adhesions and preventing them from re-forming.

If this part is insufficient, the multiple folds will eventually recur, leading to the return of triple or multiple eyelid creases.
