This post discusses how to correct asymmetrical eyes using incisional ptosis correction when one eye appears sleepy.

This patient had undergone surgery in the past but was referred to me because one eye looked sleepy.

As seen in the pre-operative photo, the left eye looks slightly sleepy.

At the same time, the crease on that side appears higher. This is because the eye is not opening fully, making more of the crease visible.

In reality, the crease height is the same when the eyes are closed.

When the eyes are closed, you can see the creases are identical. The patient mentioned they previously had the buried suture method. Even if the double eyelid creases are made identical, if the muscle strength used to open the eyes differs, the eyes will look asymmetrical and the crease sizes will appear different, as shown in the photo above.

This is the appearance one week after incisional ptosis correction. Symmetry has been mostly achieved, though some bruising remains.

The patient requested to match the appearance of their right eye, and the crease height seems to have turned out well without being too high.
