Raising the double eyelid line: Changing from an in-fold to an in-out fold (tapered fold -> neutral fold)

After undergoing double eyelid surgery, patients often initially request a smaller line because they are hesitant about a dramatic change.

In other cases, the line may begin to sag over time as the years pass after the initial surgery.

What if the line itself is too low?

In such cases, it is recommended to raise the line.

In-fold before surgery

In-out fold after surgery (1 week post-op)

A new incision is made above the existing scar, and the lower scar tissue is released. The upper arrow indicates the line where stitches were removed immediately after surgery, and the arrow directly below it points to the original scar. If you look closely, you can see a faint white solid line.

However, if the skin itself has sagged, it is often better to leave the line as it is and perform a skin excision instead.

Of course, both methods can be performed simultaneously.

If you excessively raise the line without excising skin, it can result in ‘sausage eyes’ and a sleepy appearance. In such cases, it is actually better to lower the line.
