First, a quick note on terminology: while often misspelled in searches, the correct terms are ‘double eyelid surgery’ and the ‘buried suture method.’
I would like to expand on my previous posts regarding this topic.
One of the greatest advantages of the buried suture method is that the surgery is simple and can be reversed if the results are not to your liking.
However, the downside is that surgeons with limited experience may find it difficult to actually locate and remove the sutures.
It can truly be like ‘finding a needle in a haystack.’

Typically, I create two tiny needle-sized holes to locate the stitches. This requires a high level of technical skill.
As seen in the photo below, I locate the sutures through two small punctures to ensure they are removed without damaging the surrounding tissue.
In cases involving non-incisional ptosis correction, it is sometimes necessary to locate the sutures separately through the conjunctiva.
Finally, to minimize scarring, I only apply a single stitch to the tiny needle-sized openings.
The marks are barely visible.
In this specific case, the patient only wanted the sutures removed and did not wish to create a new double eyelid line.
My previous post featured a case where the sutures were removed and a new line was subsequently created.
The procedure takes approximately 15 minutes. While it can be done easily with the right expertise, it is an incredibly difficult surgery without the proper technique.

Can double eyelids created with the buried suture method be reversed?
Yes, it is possible. A major advantage of the buried suture method or natural adhesion method is that if you are not satisfied, the inserted thread can be removed to restore your eyes to their original state. However, skilled technique is required to find the thread without damaging the tissue.
Will there be many scars after double eyelid reversal surgery?
No, there will be almost no scars. Usually, only two tiny needle-sized holes are made to find and remove the thread. To minimize scarring, only one stitch is lightly sutured, so the marks are not very visible after surgery.
Is thread removal possible even after non-incisional ptosis correction?
Yes, it is possible. However, unlike general buried suture thread removal, in cases where non-incisional ptosis correction has been performed, it may sometimes be necessary to find and remove the thread through the conjunctiva inside the eye. This also requires a high level of skill.
How long does double eyelid thread removal surgery take?
The surgery takes approximately 15 minutes. If performed by a specialist with skilled technique, it can be completed relatively quickly and simply, but if the experience is insufficient, it can be very difficult to find the thread, making the surgery very challenging.
Can a new double eyelid line be created after removing the thread?
Yes, it is possible depending on the patient's choice. You can either remove only the existing thread to return to your original eyes, or you can undergo surgery to create a new double eyelid line immediately after the thread removal surgery.
