Upper Blepharoplasty for Middle-Aged Patients: Addressing Sagging Eyelids
In the fall of 2017, I gave a presentation at the ‘Korean Society of Plastic and Reconstructive Surgeons’ regarding #Upper Blepharoplasty and #Ptosis for the elderly.
Often, middle-aged patients can look sharp or unnatural after eye surgery. The area under the eyebrows where the double eyelid is formed can also appear bulky. My presentation focused on how to solve these issues and achieve the most natural look possible, as these are common concerns. Since older patients have significant skin sagging, it is difficult to achieve the exact same results as someone in their teens or 20s. However, these patients aren’t necessarily looking to look like they are in their 20s; the goal is to remove the sagging look and make them appear naturally younger—by about 10 years. For those with sagging on the outer corners, as shown below, it is effective to carefully adjust that area.
For those with asymmetry, correcting it leads to a much better aesthetic result. In the photo below, the right eye is smaller and the crease line is lower. In such cases, performing #PtosisCorrection is very helpful.

The main goal is to make you look about 10 years younger than your current age. As skin sags significantly with age, the focus is on creating natural-looking eyes without a droopy appearance, rather than trying to make them look like a teenager's or someone in their twenties.
Can middle-aged eye surgery make my impression look fierce?
Yes, it is common for people to look fierce or unnatural after surgery. In particular, the double eyelid area under the brows can appear thick, so it is very important to perform the surgery to prevent this and ensure the most natural look possible.
How is surgery performed if the outer corners of the eyes are significantly saggy?
If there is severe sagging in the outer corners, that area is specifically adjusted and corrected. By appropriately trimming the sagging skin on the outer corners of the eyes, a much more natural and youthful eye shape can be achieved.
What if the size of both eyes or the double eyelid lines are different?
Asymmetry can be corrected through ptosis correction surgery. For example, if the right eye is smaller or has a thinner line, combining it with ptosis correction to match the symmetry of both eyes will result in a much more aesthetically pleasing shape.
