Can Upper and Lower Blepharoplasty Be Performed Together?

In this patient’s case, there was sagging in both eyes, a slightly sleepy appearance, and asymmetrical eyelid lines.
For older patients, we generally avoid excessive ptosis correction for ‘sleepy eyes.’
This is because making the eyes too large can result in a ‘staring’ look or an unnatural expression.

This is the appearance after surgery. The sleepy look has been well-corrected, and the eyelid lines have been changed to look natural.
The bulging areas under the eyes were also improved simultaneously.
As a note, at our clinic, if a patient originally has ‘aegyo-sal’ (under-eye rolls), we perform the surgery while carefully preserving the nerves so that they can reappear.
However, if a patient does not naturally have aegyo-sal, it will not be created by the surgery.
Many people worry whether undergoing both upper and lower eyelid surgery (upper and lower blepharoplasty) at the same time will look awkward or if the swelling will last significantly longer.
Based on our clinic’s standards, there is no major difference. Stitches are removed within the same 5 to 10-day window.
